District Wide Student Registration
Print out forms and return during district wide student
registration on August 12, 1:00-7:00 pm and August 13, 8:00-4:00
pm in the High School Cafeteria.
Several changes have been made on district forms.
Please read through forms first and then fill out completely.
Forms 1 - 6 are required for each household.

Medication form-one per student
Form 9 is required for all 7 - 12 students participating in
co-curricular activities. One form is required per
household.
